Your Appointment - When you make your first appointment, you will be enrolled in the BSW​ Elder HouseCalls program. You will be assigned a nurse practitioner located in your region. Your nurse practitioner will call you 24 hours in advance to set up a time for your home visit.

We strongly recommend that you complete the new patient packet and advanced directive forms BEFORE your visit. There can be faxed, mailed, or physically brought to our office.

During your home visit, your nurse practitioner will perform a comprehensive geriatric assessment and review with you any medications you are taking. If you have been under the care of another physician, we may ask your permission to transfer some of your records. 

Making Appointments - We have three appointment categories that take up varying amounts of time:

  • New Patient Visits - Plan on spending one hour with your nurse practitioner during your first home visit
  • Follow-up Exam - These are done every 30 days by your nurse practitioner and may take up to 30 minutes depending on your current health status
  • Sick Visits - If you feel that you are sick and require a home visit, you may call our office to schedule an appointment. Your nurse practitioner will visit you within 24 hours of your call. We also have a physician that is on-call 24 hours.
