Enrolling in BSW HouseCalls

To enroll in the BSW HouseCalls program, you must be UNABLE TO TRANSPORT YOURSELF to a primary care provider.

The BSW HouseCalls program accepts referrals from the following:

  • Self Referral
  • Nursing Home/Senior Center/Other Care Facility 
  • Primary Care Physician 

Upon enrollment, you will be assigned a Nurse Practitioner in your region. Our Nurse Practitioners make every effort to see new patients within 7 days of your enrollment. If you are referred by a hospital or other care facility, visits will be conducted within 72 hours of enrollment.

Once enrolled, you will receive a visit from your assigned Nurse Practitioner every 30-90 days in an effort to manage your health care needs. In addition, you will be seen periodically by a physician who is familiar and involved in your individual care plan.

BSW​ HouseCalls program hours of operation are from 9:00 a.m. to 5:00 p.m. with 24/7 telephone coverage.

What to Expect at Your First Visit

A comprehensive geriatric assessment will be completed that effectively addresses prevalent problems faced by elder patients as well as the many other areas of geriatric care that are crucial to the successful treatment and prevention of disease and disability in our elderly patients. 

When meeting with one of our providers, some of all of the following areas may be addressed:

  • Current symptoms and illnesses and their functional impact
  • Current medications; their indications and effects
  • Relevant past illnesses
  • Recent and impending life changes
  • Objective measure of overall personal and social functionality
  • Current and future living environment and its appropriateness to function and prognosis
  • Family situation and availability
  • Current caregiver network including its deficiencies and potential
  • Objective measure of cognitive status
  • Objective assessment of mobility and balance
  • Rehabilitative status and prognosis if ill or disabled
  • Current emotional health and substance abuse
  • Nutritional status and needs
  • Disease risk factors, screening status, and health promotion activities
  • Services required and received
